We haven't gotten confirmation of any cities outside the 3 main ones, just rumors that there could be others. As of now I would bet that there will be absolutely zero fighting on the streets in these main cities. In the end we're still just speculating for the most part... but those are my best guesses.

Oh and as for that crazy mission idea, what if the people shooting didn't actually kill the "escapees", but they still got XP credit based on how much damage they did? That way it still gives the intense atmosphere for the runners without them getting slaughtered, and still gives a reason for people to even bother shooting. The question then of course becomes how much xp you wanna give for a freebie mission like that in the first place, and whether any higher levels would even bother since it likely wouldn't do much toward leveling compared to PVP'ing.
I'm pretty sure they've said that aside from the 3 main cities (2 of which are for the playable races) there will be 8 smaller cities in which the fighting will take place. They have also said that there will be no fighting in the main cities (I'm guessing you just can't shoot at all). As to city guards, there might be some just for looks and maybe some more just in case some enemies get too close to the city (I have no idea but that would be cool if you could kind of attack someone elses capital, even if you just sit outside shooting at anyone who comes out).

As to the mad dash under heavy high level fire. If they ever wanted to put something like that into the game (probably not, but I think it would be really fun for both sides lol). They could have the high levels pick up the mission, then everyone in the general area gets grouped together in the quest all at the same time (kind of like the quest system that warhammer: age of reckoning will have). I would say that the low levels should respawn right where they died if not a little further ahead in the run (just so they can always make it to the fort no matter what). This way the higher levels are pretty much just giving them a hard time for a bit, but not really effecting the game too much. As for exp. they could just give a general amount of exp. to everyone who participated.
