With regards to spotting them.. the max capacitor hack might be difficult but if its used to such an extent as to make it unfair then it becomes obvious.
The CoF hack is obvious.. i can spot someone using it for example when crouched and firing in bursts using a medium assault rifle and someone on the otehr team is moving and firing in full auto and still is more accurate than me, in regards to the forced innacuracy through the CoF.
Once someone is caught by a player they will most likely get banned, problem is they have to wade through large numbers of reports generated by bad losers who dont believe anyone can beat them. Other problem is they can then make a new account for free and carry on doing it.

Well yes, the speedhack is very obvious. Trust me though, the CoF hack is very hard to detect. The majority of times when you think someone is using it, you can't prove it.
The way the CoF hack works is: It does not remove the CoF. It freezes the CoF at at the size it is at when you press the button to make it do that. The effects of the hack itself cannot be seen by monitoring someone's account. It can only be found if the Trainer Program itself is detected.
